Four years on and the story remains the same. This document, which also heralds the merger between SmithKlineBeetcham and Glaxo, reiterates the 1997 fax. Nobody has noticed that they have a problem

Package Inserts 1971
So having got this far, we now know that Glaxo, in whichever guise are saying the same, thing, "We warned everybody about the dangers of Arachnoiditis on the 1971 Package Inserts".
Here we publish the relevant sections of the Pakage Inserts which form their defence against culpability. Note the differences before moving on to our final document in this series.

The Doctor tells the tale - 1972

Replying to various concerned hospital doctors and the Committee on the Safety of Medicines, he attempts to explain away instances of Adverse Drugs Reactions following Myodil myelograms.
Please note that he never brings the readers' attentions to those all important changes on the 1971 Package Inserts.

Authored again by Dr Snell of Glaxo this was sent to the CSM listing the Adverse Drug Events involving Myodil known to the Company. The reader should note that there was no appropriate or reliable ADR reporting system at that time.
